If you’re a current patient, you can request an appointment via our patient portal, Patient Fusion.
Not sure if you’ve signed into the Patient Fusion before?
If you have not signed into the portal previously, please check your email for the invitation to the portal called Patient Fusion.

Getting Started with Patient Fusion
After your first appointment has been conducted, Patient Fusion automatically generates an invitation and sends it to your email address on file.
- Open the email.
- Click on the orange button that says, “Access Your Records.”
- Finishing setting up your account.
- Confirm your email. After completing the sign-up process, you will receive a confirmation in your email. Proceed to login with the credentials you created.
- Verify your enrollment. Log into Patient Fusion and register using one of the following methods:
- Using the temporary PIN (Call the office if you need a PIN)
- Using your phone number recorded by the doctor
Tip: If you didn’t receive an invitation to Patient Fusion in your email, check your Promotions tab in Gmail.

Are you a new patient?
The patient portal is for current patients only. However, you can head over here to request a new patient appointment for 18+ or head over here to request an appointment for new patient under 18.
